Step by Step with Screenshots in findhelp.org
Step 1
Go to findhelp.org and enter your patient’s zip code.
Step 2
Search “Women, Infants and Children” in the search bar
Click “Women, Infants, and Children (WIC) once it appears
Step 3
Find your patient’s preferred WIC location and select “APPLY HERE”
Step 4
Click “I’m referring someone else” fill out the boxes with your info.
Step 5
Fill out all of the fields with your patient’s information.
Step 6
Check the “Confirm Consent” box and click “NEXT”
Step 7
You will be taken to a new page to fill out all the info on the page.
Step 8
Upload any necessary medical documents.
Step 9
A summary page will appear, verify the info and click “Submit”
